Apply some uncommitted changes from 25 Dec 2007 (!). Details:
- release_rpcs slightly refactored to have its reason pass in to it
- release_rpcs called in two new cases: 'EXIT's both with and without
an AMQP-specific exit reason
- channel.close forbidden as an explicit action.
